Please complete the form below to request your repeat prescription medication. This form can only be used for medications which you are prescribed as repeat medication on an ongoing basis from your GP at the surgery.

We often receive a high volume of prescription requests – please be aware that your prescription may take up to 48 hours to be issued, though we will endeavour to have it actioned as soon as possible. Should you wish to be notified when your prescription is ready, please select this option in the form below.

In line with CQC guidelines, our Doctors will issue a maximum of 3-months of medication per prescription. Repeat prescriptions will require a medication review with your doctor. How frequently this occurs depends on the medication prescribed and your medical history. At a minimum, for any repeat prescription, you will need to be seen annually.
